The Medicare Modernization Act of 2003, requires Medicare Part D prescription drug plans to include medication therapy management services delivered by a qualified healthcare professional, including pharmacists, beginning in 2006. MTM services target beneficiaries who have multiple chronic conditions, take multiple medications, or are likely to incur annual costs above a predetermined level.

Providing comprehensive medication reviews in depression, bipolar disorder, anxiety, PTSD, schizophrenia, smoking cessation, substance abuse. 

We offer services targeting mental health discharge follow up, primary care behavioral, formal and informal psychopharmacology consults, clozapine management, medication reconciliation and non-formulary medication consults.

Services include medication dosing and treatment recommendations, drug-drug interaction information, patient education groups, and individual patient evaluations for medication-related issues.

Pharmacists may initiate, change or discontinue medications and obtain laboratory tests; also reviews laboratory results, obtains medication histories and provides medication education and coordination of care.

Pharmacists provide patient education, check adherence, help determine appropriateness and effectiveness of medications regiment, identify and prevent medication-related problems, minimize polypharmacy, dispense needed medications, and/or notify prescribers if problems ensue with medication.
